Insurance Information
Before scheduling, please verify that your health plan includes Maren Medical Group. Check our Insurance and Medicare pages for up-to-date lists of accepted insurance providers.
You’ll need to designate your chosen physician as your primary care provider (PCP) with your insurance company. Each doctor’s profile page includes their provider ID numbers for various health plans to make this process easier.

Transferring Your Medical Records
We can help securely transfer your records from previous providers. Simply complete our Medical Records Release Form (available here on our website or at your first visit) and we’ll handle the rest.
If your previous provider uses MyChart, the transfer becomes even more streamlined, allowing your new physician quick access to your medical history.
