Manage your family’s care

MyChart provides secure online access to your health information and convenient tools to manage your care.

Patients can sign up for MyChart in several ways:

  • During your visit at the front desk or in the exam room
  • Using an activation code from your After Visit Summary
  • Requesting an activation code (delivered by mail) using a paper form

You may also be able to create an account online by verifying your identity against your medical record or through third-party verification.

E-Visit Messaging
Talk to your health team without an office visit through secure messaging. To begin, log in to MyChart and select Your Menu > Start an E-Visit Message with a Virtual Care Provider.

Video Visits
Connect with your provider from a convenient location using video. You’ll need an active MyChart account to participate, using either a mobile device or computer. Your provider will determine if your situation is appropriate for a video visit.

Video Visit Tips

  • Position yourself away from windows or bright lights for better video quality
  • The Begin Video Visit button appears 45 minutes before your appointment
  • You can eCheck-in up to three days before your appointment
  • If connection issues occur, simply reconnect

For eligible appointments, complete these tasks before you arrive:

  • Pay copays and balances
  • Verify or update insurance and personal information
  • Update medications, allergies and health issues
  • Complete appointment questionnaires

Changing or Adding A Provider

If you already have a MyChart account, you can add Maren or even connect another medical provider.

MyChart To-Go

Take MyChart with you by downloading the app on your mobile device. Access lab results, appointments, medications, and immunization records on the go through the App Store or Google Play.
