If your plan isn’t listed yet, let us know at coverage@maren.com—or ask your insurance provider directly. And when you’re ready to switch, use your doctor’s provider ID to smooth the way.

We’ve found that patient advocacy helps move things along much quicker. If your plan isn’t listed yet, we HIGHLY encourage you to contact your insurer directly and emphasize your desire to see a physician with LaSalle Medical Associates/Maren Medical Group. Your voice matters.

Use Provider IDs when updating HMO and Advantage plans

Change to Maren with confidence. If you have an HMO or Medicare Advantage plan, use your doctor’s provider ID when contacting your insurer to ensure a seamless switch to Maren. Select your doctor below.

Note: We add IDs for providers as they become available. But even if your health plan’s provider ID is listed as coming soon, you can still change to Maren. Just call us at (909) 312-0085 and we’ll be happy to help.

What’s a provider ID?

Unique insurance provider IDs are assigned by insurance companies to each doctor in an organization. When a doctor changes to a new medical group, they get a new provider ID, too.

Some types of plans (like PPOs) don’t rely on provider IDs. For others (like HMOs and Medicare Advantage plans), provider IDs are a smart way to make sure you and your health plan are talking about the same doctor at the same place when updating your insurance.
